Nebraska's corrections director faced tough questions about how convicted quadruple killer Nikko Jenkins is being handled in prison.

Omaha Senator Ernie Chambers grilled Corrections Director Scott Frakes about how Jenkins was able to swallow a prison employee's keys while under around-the-clock supervision. Jenkins has also cut his face, tongue, and genitals while awaiting sentencing for four murders in Omaha in August 2013.

Frakes defended his staff's actions but admitted workers did make mistakes in their handling of Jenkins, but said no one has been fired. Frakes' comments came during a legislative hearing on the state's prison system, which touched on issues such as staff turnover and the use of solitary confinement.
